Antecedentes y objetivo: El síndrome aórtico agudo (SAA) es poco frecuente y difícil de diagnosticar, con una gran variabilidad en su cuadro clínico inicial. Los objetivos son: 1) desarrollar un algoritmo informático, o un sistema de apoyo a las decisiones clínicas (SADC), para el manejo y la solicitud de estudios de diagnóstico por imagen en el servicio de Urgencias, en concreto de una tomografía computarizada (TC) de la aorta, ante la sospecha de SAA, 2) determinar el efecto de la implantación de este sistema, y 3) determinar los factores asociados a un diagnóstico radiológico positivo que mejoren la capacidad predictiva de los hallazgos de la TC de aorta. Material y métodos: Tras desarrollar e implementar un algoritmo basado en la evidencia, se estudiaron casos de sospecha de SAA. Se utilizó el test de la χ2 para analizar la asociación entre las variables incluidas en el algoritmo y el diagnóstico radiológico, con 3 categorías: sin hallazgos relevantes, positivo para SAA y diagnósticos alternativos. Resultados: Se identificaron 130 solicitudes; 19 (14,6%) tenían SAA y 34 (26,2%) tenían otra patología aguda. De las 19 con SAA, 15 habían sido estratificadas como de alto riesgo y 4 como de riesgo intermedio. La probabilidad de SAA era 3,4 veces mayor en los pacientes con aneurisma aórtico conocido (p=0,021, IC del 95%: 1,2-9,6) y 5,1 veces mayor en los pacientes con un soplo de insuficiencia vascular aórtica de novo(p=0,019, IC del 95 %: 1,3-20,1). La probabilidad de tener una enfermedad aguda grave alternativa fue 3,2 veces mayor en los pacientes con hipotensión o choque (p=0,02, IC del 95 %: 1,2-8,5). Conclusión: El uso de un SADC en el servicio de Urgencias puede ayudar a optimizar el diagnóstico del SAA. Se demostró que la presencia de un aneurisma aórtico conocido y de insuficiencia valvular aórtica de nueva aparición aumentan significativamente la probabilidad de SAA. Se necesitan más estudios para establecer una regla de predicción clínica.(AU)

This study investigates the role of floating plastics as integrative samplers of organic contaminants. To this end, plastics items were collected in two Western Mediterranean coastal areas: the Mar Menor lagoon, and the last transect of Ebro river. Floating plastics were identified and characterized by attenuated total reflection Fourier-transform infrared spectrometry. Then, organic contaminants were extracted from plastic items by ultrasonic extraction with methanol, and the concentrations of 168 regulated and emerging contaminants were analysed. These compounds were analysed by stir bar sorptive extraction coupled to gas chromatography-mass spectrometry (GC-MS), except for bisphenol analogues, which were analysed with a ultraperformance liquid chromatography pump coupled to a triple quadrupole mass spectrometer (UHPLC-MS/MS), and pharmaceutical compounds, determined by UPLC coupled to hybrid triple quadrupole-linear ion trap mass spectrometer (UPLC-MS/MS). All the contaminants groups considered were detected in the samples, being particularly relevant the contribution of plastic additives. The most frequently detected contaminants were UV-filters, PAHs, pharmaceuticals and synthetic musks. Apart from plasticizers, the individual contaminants octocrylene, homosalate, galaxolide, salycilic acid and ketoprofen were frequently detected in plastics items. The results pointed out to urban and touristic activities as the main sources of pollution in the coastal areas investigated. The utility of floating plastics as integrative samplers for the detection of organic contaminants in aquatic ecosystems has been demonstrated.

Introducción y objetivos Los programas de telemedicina entre clínico y paciente se han desarrollado con fuerza durante la pandemia de enfermedad por COVID-19, pero no hay datos de experiencias entre clínicos. Nuestro objetivo es analizar el impacto de la pandemia por COVID-19 en la actividad y resultados en salud de un programa de consulta electrónica universal (e-consulta) para todas las derivaciones de pacientes entre médicos de atención primaria y el Servicio de Cardiología de nuestra área. Métodos Analizamos mediante regresión logística a 25.121 pacientes con al menos una e-consulta entre 2018 y 2021 realizada con el Servicio de Cardiología de nuestra área sanitaria. También se realizó el análisis de regresión logística del impacto de la pandemia por COVID-19 sobre la resolución de la e-consulta y tiempo de espera de la atención, hospitalizaciones y mortalidad, tomando como referencia las consultas realizadas durante 2018. Resultados Observamos que una menor demora en la atención y resolución de la e-consulta (sin necesidad de atención presencial) se asociaba a un mejor pronóstico. Los períodos de pandemia COVID-19 presentaron similares resultados a los del 2018. Conclusiones Los resultados de nuestro estudio muestran una significativa reducción de las derivaciones a través de e-consulta durante el primer año de la pandemia por COVID-19 con recuperación posterior de la demanda asistencial sin que los períodos de pandemia se asociasen con peores resultados en salud. La reducción del tiempo de demora de resolución de la e-consulta y el grupo sin necesidad de consulta presencial se asociaron a un mejor pronóstico (AU)

Conclusions The results of our study show a significant reduction in e-consult referrals during the first year of the COVID-19 pandemic with a subsequent recovery in the demand for care without the pandemic periods being associated with worse outcomes. The reduction in the time elapsed for solving the e-consult and no need for in-person visit were associated with better outcomes (AU)

BACKGROUND: Increased C-reactive protein (CRP) is used to diagnose and predict response to treatment in acute severe ulcerative colitis (UC). AIMS: To investigate the connection between CRP elevation and deep ulcers in UC. METHODS: Patients with active UC were enrolled in a multicenter prospective cohort and a retrospective cohort of consecutive patients undergoing colectomy from 2012 to 2019. RESULTS: Forty-one (9 (22%) with deep ulcers) patients were included in the prospective cohort: 4/5 (80%) patients with CRP > 100 mg/L, 2/10 (20%) patients with CRP between 30 and 100 mg/L and 3/26 (12%) patients with CRP < 30 mg/L had deep ulcers (p = 0.006). In the retrospective cohort [46 patients (31 (67%) with deep ulcers)], 14/14 (100%) patients with CRP > 100 mg/L, 11/17 (65%) patients with CRP between 30 and 100 mg/l and 6/15 (40%) patients with CRP < 30 mg/L had deep ulcers (p = 0.001). Positive predictive value of CRP > 100 mg/l for presence of deep ulcers was 80% and 100% in both cohorts, respectively. CONCLUSIONS: CRP elevation is a robust surrogate marker for presence of deep ulcers in UC. Elevated CRP or presence of deep ulcers could influence the choice of medical therapy in acute severe UC.

The increase in energy and fertilizer consumption makes it necessary to develop sustainable alternatives for agriculture. Anaerobic digestion and digestates appeared to be suitable options. However, untreated digestates still have high water content and can increase greenhouse gas emissions during storage and land application. In this study, manure-derived digestate and solid fraction of digestate after separation were treated with a novel solar drying technology to reduce their water content, combined with acidification to reduce the gaseous emissions. The acidified digestate and acidified solid fraction of digestate recovered more nitrogen and ammonia nitrogen than their respective non-acidified products (1.5-1.3 times for TN; 14 times for TAN). Ammonia and methane emissions were reduced up to 94% and 72% respectively, compared to the non-acidified ones, while N2O increased more than 3 times. Dried digestate and dried acidified digestate can be labeled as NPK organic fertilizer regarding the European regulation, and the dried solid fraction and the improved dried acidified solid fraction can be labeled as N or P organic fertilizer. Moreover, plant tests showed that N concentrations in fresh lettuce leaves were within the EU limit with all products in all the cases. However, zinc concentration appeared to be a limitation in some of the products as their concentration exceeded the European legal limits.

Introducción y objetivo: La ansiedad específica del cáncer (CSA) es la reacción psicológica más frecuente tras la prostatectomía radical (PR). Evaluamos la prevalencia de la patología psiquiátrica pretratamiento de cáncer de próstata mediante PR e identificamos la influencia de los diagnósticos psiquiátricos en la supervivencia y pronóstico en los pacientes. Material y métodos Estudio retrospectivo multicéntrico observacional, 1.078 varones intervenidos mediante PR por cáncer de próstata órgano-confinado. Grupos: GP: pacientes con patología psiquiátrica previa a la PR; GNP: pacientes sin patología psiquiátrica previa a la PR, variables urológicas, oncológicas y psiquiátricas, estadística descriptiva y análisis multivariante. Resultados El 37,94% presentó algún diagnóstico psiquiátrico. Fue necesario tratamiento adyuvante de radioterapia (RT) en 27,83% y hormonoterapia (HT) en 23,38%; más frecuentes en GP. La supervivencia cáncer-específica fue superior en GNP. La ansiedad, depresión, insomnio, tabaquismo, psicosis y alcoholismo fueron los más frecuentes. La baja estadificación Tumor-Ganglios-Metástasis (TNM) y poca presencia de síntomas del tracto urinario inferior (STUI) e incontinencia urinaria de esfuerzo (IUE) incrementó la probabilidad de ausencia de patología psiquiátrica. En GP aumentó la fatiga, disfunción eréctil y deterioro cognitivo tras la PR junto con RT y/o HT. A mayor edad y mayor antígeno prostático específico (PSA) al diagnóstico, aumentó el riesgo relativo de patología psiquiátrica y peor evolución. Los factores más relacionados fueron la PR, PSA, la edad y el tiempo de supervivencia. Conclusiones La patología psiquiátrica está presente en pacientes tratados mediante PR debido a cáncer de próstata, teniendo alto impacto en los resultados de supervivencia y pronóstico (AU)

INTRODUCTION: Enhanced Recovery After Surgery (ERAS) protocols and educational programmes have been shown to accelerate orthopaedic surgery recovery with fewer complications, and improve patient-reported outcomes (PROs) for different types of surgery. The objective was to evaluate the impact of an ERAS programme including a patient school on health outcomes and PROs for Total Knee Replacement (TKR) surgery. MATERIAL AND METHODS: A multidisciplinary group created the programme and the patient school (preoperative consultations where the patients' surgical processes are explained and are also given instructions for an appropriate perioperative care management). An observational, prospective study was conducted on all patients operated for TKR from March 2021 to March 2022. Main health outcomes were: hospital stay length, surgical complications and surgery cancellations due to a wrong preoperative medication management. PROs evaluated were: patient satisfaction with pain management, the school, and quality of life before and after surgery (EQ-5D). RESULTS: One hundred thirty-three patients were included. Median hospital stay length was 3 days (IQR 3-5). Rate of surgical complications was 25.6%. No surgery was cancelled. Patient satisfaction rates with pain management and with the school were 8.10/10 and 9.89/10, respectively. Concerning quality of life, mean improvement in mobility and knee pain after the surgery was 0.66 (p < 0.05) and 0.84 (p < 0.05), respectively. CONCLUSIONS: The ERAS programme including a patient school was highly successful with a fast recovery, a short hospital stay length, no surgery cancellations, and improved PROs.

INTRODUCTION: The study of Paternal-fetal attachment is a recent research area, which has become relevant due to its correlation with the postnatal paternal bond and its impact on the cognitive and socio-emotional de velopment of the child. OBJECTIVE: to characterize Paternal-fetal attachment, based on maternal pers pective, in puerperal women treated in a public hospital in the Metropolitan Region. SUBJECTS AND METHOD: A cross-sectional observational study was carried out. Hospitalized puerperal women were interviewed by applying a questionnaire with sociodemographic variables of both parents. In addi tion, the Paternal-fetal attachment Scale from the maternal perspective was applied, previously adap ted and validated. A descriptive and association analysis was performed to determine the behavior of the link indicator with respect to the sociodemographic variables. RESULTS: The mean Paternal-fetal attachment score from the maternal perspective was 41.5 (range 10-50), which is significantly higher in cases where parents live together, the couple's relationship is longer than two years, and in plan ned and controlled pregnancies before twelve weeks. There were no differences in the Paternal-fetal attachment score from the maternal perspective in relation to the number of children, nationality, or educational level. DISCUSSION: The Paternal-fetal attachment is mainly determined by characteristics of the parental relationship and not by the individual characteristics of the parents, therefore, the attachment must be seen from the triad.

INTRODUCCIÓN: la erupción del Volcán de Cumbre Vieja en La Palma. "Este desastre natural ha acarreado a lo largo de tres meses de actividad la muerte de una persona, el desplazamiento de más de 7.000 en una comunidad de algo más de 80.000 residentes, la desaparición de más de 1.200 edificaciones que incluyen viviendas, empresas, colegios, iglesias y hasta cementerios, más de 1.000 hectáreas de terreno de cultivo y la destrucción o deterioro de infraestructuras vitales como carreteras, conducciones eléctricas e hidrológicas. El día a día se ha acompañado de terremotos cotidianos, el rugido y el tremor volcánico, la emisión por el cráter de gases tóxicos, cenizas y piroclastos y el progreso arrasador de la lava incandescente a través de distintas coladas. Evidentemente, esta situación de indefensión y sufrimiento puede originar consecuencias en la salud mental de los afectados, especialmente aquellos más vulnerables como personas con patologías previas, escaso apoyo social o que han sufrido un daño más significativo, tal como se ha visto en otras catástrofes, con la previsible aparición, más o menos diferida, de trastornos como el estrés postraumático, la ansiedad, la depresión o el aumento en el consumo de alcohol y sustancias”.OBJETIVOS: poner de manifiesto, a través de la presencia de los farmacéuticos comunitarios (FC) de Canarias y la acción profesional en las áreas de salud de la carpa, el compromiso de la Sociedad Española de Farmacia Clínica, Familiar y Comunitaria (SEFAC) con la población afectada en la Isla de La Palma con motivo de la erupción del Volcán de Cumbre Vieja del pasado 19 de septiembre de 2021. Ofrecer servicios de educación sanitaria sobre los trastornos de salud derivados de la erupción más prevalentes detectados en las farmacias comunitarias de la isla a través de la labor humanística de escucha, apoyo y motivación del FC como profesional sanitario. JUSTIFICACIÓN: los problemas asociados a la calima y el impacto del polvo sahariano son un potencial peligro para la salud, y se manifiestan la mayoría en síntomas menores respiratorios y oftalmológicos tales como congestión nasal, tos, ojo rojo y ojo seco. Aunque la duración habitual es de 3 a 5 días, en las Islas Canarias ocurrió un episodio del 27 de enero de al 12 de febrero de 2022. Valor en la práctica habitual del Servicio definido y protocolizado por Foro de Atención Farmacéutica en farmacia comunitaria, siendo necesario: reducir la inseguridad del paciente en el autocuidado y la automedicación, en el caso de los síntomas menores realizar actuación o intervención: dispensación del medicamento solicitado que no precise prescripción, asegurando que es utilizado correctamente. Indicación de tratamiento farmacológico que no precise prescripción, para ese síntoma menor. Indicación de medidas higiénico-dietéticas, educación sanitaria y/o recomendaciones sanitarias. La derivación al centro de salud de manera adecuada y consensuada, potenciando la comunicación con otros profesionales sanitarios implicados en la atención a ese paciente.MATERIAL Y MÉTODOS: reunión de los tres farmacéuticos comunitarios en plataforma ZOOM para coordinar la recopilación de datos en el SIF de los siguientes síntomas menores: congestión nasal, tos, ojo rojo y ojo seco. Creación de documento compartido en la plataforma Google Drive donde poder registrar la información. Entrevista con el paciente: PASITAMAE. Registro del servicio en Programa Digital de SEFAC, que permite la evaluación simultanea del paciente con ayuda de los protocolos consensuados. Entrega del Informe para el paciente de la actuación farmacéutica y/o entrega del informe de derivación al médico. Cuantificar los datos de las tres farmacias comunitarias en función de resolución del síntoma menor y/o derivación al médico de Atención primaria o al Servicio de Urgencias. (AU)

This study evaluates the relationship between gender, personality, psychopathology and suicidal behaviour among adolescents. For this purpose, across-sectional study with adolescent inpatients (N = 92) displaying suicidal behaviour was designed. Sociodemographic characteristics and datarelated to suicidal behaviour were collected. In addition, personality and clinical severity were assessed by administering the Millon Adolescent Clinical Inventory (MACI). Results revealed that the most prevalent personality dimensions for both genders were Introversive, Self-demeaning, Doleful,Oppositional, Borderline tendency and Inhibited. However, men scored higher in Inhibited (p = 0.02, Cohen’s d = 0.45) and women in Egotistic(p = 0.03, Cohen’s d = 0.40). Concerning clinical severity of syndromes, women scored higher in Delinquent Predisposition (p = 0.04, Cohen’sd = 0.45) and men in Anxious Feelings (p = 0.02, Cohen’s d = 0.51), Depressive Affect (p = 0.04, Cohen’s d = 0.41) and Suicidal Tendency (p= 0.03, Cohen’s d = 0.37), with men reporting more frequently Childhood Abuse (p < 0.001, Cohen’s d= 0.70). Results highlight the importanceof universal interventions to promote a change in attitudes towards seeking psychological help, specially in men, and the need to designeffective tailored treatments to acquire emotional management skills for both genders. Este estudio evalúa la relación entre el género, lapersonalidad, la psicopatología y la conducta suicida entre los adolescentes. Para ello, se diseñó un estudio transversal con adolescentes hospitalizados (N = 92) que presentaban conductas suicidas. Se recogieron características sociodemográficas y datos relacionados con la conductasuicida. Además, se evaluó la personalidad y la gravedad clínica mediante la administración del Inventario Clínico de Adolescentes de Millon (MACI).Los resultados revelaron que las dimensiones de personalidad más prevalentes para ambos géneros fueron Introversivo, Auto-punitivo, Pesimista,Oposicionista, Tendencia límite e Inhibido. Sin embargo, los hombres puntuaron más alto en Inhibido (p = 0,02, d de Cohen = 0,45) y las mujeresen Egocéntrica (p = 0,03, d de Cohen = 0,40). En cuanto a la gravedad clínica de los síndromes, las mujeres puntuaron más alto en PredisposiciónDelictiva (p = 0,04, d de Cohen = 0,45) y los hombres en Sentimientos Ansiosos (p = 0,02, d de Cohen = 0. 51), Afecto Depresivo (p = 0,04, dde Cohen = 0,41) y Tendencia Suicida (p = 0,03, d de Cohen = 0,37), y los hombres informaron con mayor frecuencia de Abuso en la Infancia(p < 0,001, d de Cohen = 0,70). Los resultados ponen de manifiesto la importancia de las intervenciones universales para promover un cambiode actitud hacia la búsqueda de ayuda psicológica, especialmente en los hombres, y la necesidad de diseñar tratamientos eficaces a medidapara adquirir habilidades de gestión emocional para ambos géneros. OBJECTIVE: The objective of the study is to identify the risk factors associated with mortality at six weeks, especially by analyzing the role of antivirals and munomodulators. DESIGN: Prospective descriptive multicenter cohort study. SETTING: 26 Intensive care units (ICU) from Andalusian region in Spain. PATIENTS OR PARTICIPANTS: Consecutive critically ill patients with confirmed SARS-CoV-2 infection were included from March 8 to May 30. INTERVENTIONS: None. VARIABLES: Variables analyzed were demographic, severity scores and clinical condition. Support therapy, drug and mortality were analyzed. An univariate followed by multivariate Cox regression with propensity score analysis was applied. RESULTS: 495 patients were enrolled, but 73 of them were excluded for incomplete data. Thus, 422 patients were included in the final analysis. Median age was 63 years and 305 (72.3%) were men. ICU mortality: 144/422 34%; 14 days mortality: 81/422 (19.2%); 28 days mortality: 121/422 (28.7%); 6-week mortality 152/422 36.5%. By multivariable Cox proportional analysis, factors independently associated with 42-day mortality were age, APACHE II score, SOFA score at ICU admission >6, Lactate dehydrogenase at ICU admission >470U/L, Use of vasopressors, extrarenal depuration, %lymphocytes 72h post-ICU admission <6.5%, and thrombocytopenia whereas the use of lopinavir/ritonavir was a protective factor. CONCLUSION: Age, APACHE II, SOFA>value of 6 points, along with vasopressor requirements or renal replacement therapy have been identified as predictor factors of mortality at six weeks. Administration of corticosteroids showed no benefits in mortality, as did treatment with tocilizumab. Lopinavir/ritonavir administration is identified as a protective factor.

Dietary strategies can potentially help to reduce nitrogen (N) emissions and decrease the environmental impact of beef production. This study aimed to evaluate the effects of dietary crude protein (CP) concentration on animal performance, N excretion, and manure N volatilisation of finishing Holstein animals. In a first study, 105 Holstein bulls (BW 344 ± 2.6 kg; age 252 ± 0.9 days) were allocated to eight pens to evaluate the effect of two treatments (medium (M) and low (L), which contained CP 14.5% and 12% on a DM basis, respectively) on performance, and results confirmed that dietary CP decrease did not impair animal growth. In a second study, N excretion study, 24 Holstein heifers (BW 310 ± 5.3 kg; age 251 ± 1.4 days) were distributed randomly depending on the initial BW to three treatments (high (H), M, and L, which contained CP 17%, 14.5% and 12% on a DM basis, respectively). Based on N excretion, urinary N excretion was greater (P < 0.001) in H than in M and L diets, but no differences in faecal N excretion were observed among treatments. A third study with in vitro assays under aerobic and anaerobic conditions was designed to analyse gaseous emissions (volatilisation of N and carbon, C) during the storage stage of manure. Manure, faecal and urine samples, mixed at a ratio of 1:1 (wet weight), were collected during the N excretion study (manure-H, manure-M, manure-L). Under aerobic conditions, manure-M and manure-L showed a delay of 4-5 days in manure ammonia emission compared with manure-H (P < 0.01). Total N content was lower (P < 0.01) in manure-L compared with manure-M and manure-H, but N volatilisation (percentage relative to initial N) in manure-L and manure-M was greater (P < 0.01) than in manure-H. In contrast, the anaerobic N volatilisation was 20 times greater in manure-M and 10 times greater in manure-H compared with manure-L. Under aerobic and anaerobic conditions, the emission of C, as C-CO2 and C-CH4, was greater in manure-L than in manure-H and manure-M. Therefore, the decrease of dietary CP concentration from 17% to 14.5% and 12% is an efficient strategy to reduce urinary N excretion by 40%, without impairing performance, and also to reduce manure N losses through ammonia volatilisation under anaerobic conditions. However, a dietary CP content of 14.5% resulted in less environmental impact than a CP content of 12.8% when also considering manure emissions under aerobic or anaerobic conditions.
